There is a special symbol box attached to the top right of the enemy grid. It can randomly award a:

Hellfire Spins

Landing three of the skull Scatter symbols triggers hellfire spins, which award players five health points as they prepare to take on a boss enemy. 

Boss enemies attack at the end of each spin and will remain on the reels until they are killed. Players’ health points replenish when they defeat a boss enemy. Hellfire Spins end when the player’s health points are totally depleted or when all boss enemies are defeated. The boss enemies always appear in the same order, building from the Baron Twins to the Hellfire Tyrant. Both of the Baron Twins must be killed when they are up.

Landing three skull Scatter symbols during the feature awards a respawn feature. If the player is defeated by a boss enemy while a respawn is active, then the health points replenish, and the active boss enemy is wiped from the reels. The respawn feature does not activate during the final boss.
